What Pharmko Needs to Process a Referral


To initiate home infusion therapy, Pharmko requires:


  • Patient demographics and contact information
  • Diagnosis (ICD-10 code) and clinical indication for IV therapy
  • Medication orders , drug, dose, frequency, and duration
  • IV access type , central line, PICC, or port information can be confirmed during our first nursing visit if needed
  • Insurance information (member ID, group number, plan name)
  • Prescriber information (NPI, DEA if applicable, contact details)
  • Relevant lab results or clinical notes supporting the order


The Referral Process Step by Step


Step 1: Submit the referral


Referrals can be submitted by fax (516-670-9413), through your EHR system if integrated, or by calling Pharmko directly at 1-877-549-3863. Our team is available Monday through Friday during business hours, with 24/7 urgent support for time-sensitive discharges.


Step 2: Insurance verification and authorization

Step 3: Clinical review and patient contact


A Pharmko pharmacist reviews the prescription, confirms clinical appropriateness, and reaches out to the patient to schedule the first nursing visit and medication delivery.


Step 4: First nursing visit and therapy initiation


Our nurse visits the patient's home, confirms IV access, and trains the patient or caregiver on administration. Caregiver training and infusion pump orientation are part of our standard onboarding for every new patient.


Step 5: Ongoing coordination


Pharmko sends clinical updates to the referring provider at established intervals , or immediately if there is a change in patient status. Our team monitors labs, coordinates refills, and manages therapy adjustments in collaboration with the prescribing physician.
