Intradialytic parenteral nutrition (IDPN) is a specialized form of nutritional support delivered directly into the bloodstream during a hemodialysis session. It is designed for dialysis patients who are malnourished or at nutritional risk and are unable to meet their needs through oral intake alone.

What Is IDPN?
IDPN is a solution containing amino acids, dextrose, and lipids infused through the venous blood line of a dialysis circuit during a standard hemodialysis session. Because it runs during a treatment the patient already receives three times weekly, no additional catheter or IV access is required.
IDPN is distinct from
total parenteral nutrition (TPN). TPN is a complete nutritional replacement administered independently through a central line. IDPN is a supplemental therapy used when oral or enteral nutrition is insufficient but full TPN is not yet clinically indicated. The full spectrum of

Who Qualifies for IDPN?
Eligibility is based on documented protein-energy wasting (PEW) or malnutrition in the setting of adequate dialysis adequacy. The standard clinical criteria include:
- Serum albumin below 3.5 g/dL (or pre-albumin below 30 mg/dL)
- Kt/V ≥ 1.25 (confirming adequate dialysis delivery)
- BMI below 20 kg/m² or documented unintentional weight loss
- Inadequate response to oral nutritional supplementation after a defined trial period (typically 2–3 months)

How Is IDPN Administered?
IDPN is infused through the arterial port of the dialysis circuit at the start of the hemodialysis session and runs over the full 3–4 hour treatment. The formula is custom-compounded by Pharmko's pharmacy team based on the patient's weight, lab values, and clinical status, and is reviewed and adjusted on a regular cycle in coordination with the prescribing nephrologist.
What Clinical Evidence Supports IDPN?
Multiple studies have demonstrated that IDPN can improve serum albumin, pre-albumin, and body weight in malnourished dialysis patients. IDPN is supported by clinical guidelines from the National Kidney Foundation (KDOQI) and the American Society for Parenteral and Enteral Nutrition (ASPEN) as a nutritional intervention for patients with documented protein-energy wasting who have not responded to standard nutritional measures.

The dialysis care team or nephrologist submits a referral with supporting labs (albumin, pre-albumin, Kt/V), nutritional assessment, and dietary history. Pharmko's clinical pharmacist reviews eligibility and works with the prescribing provider to confirm the order.
Insurance authorization
IDPN is covered by Medicare Part B under specific criteria. Pharmko handles
prior authorization and submits clinical documentation, then notifies the team of coverage status before initiating therapy.
Delivery and monitoring
Pharmko ships IDPN solutions directly to the dialysis clinic on a scheduled cycle. Our team monitors labs at regular intervals and adjusts the formula based on clinical changes, communicating findings to the nephrologist and dialysis dietitian.
